Each year MOREnet seeks E-rate discounts on Internet Access and other services on behalf of eligible MOREnet members. Even if you do not apply for other E-rate services such as local and long distance phone service as an organization, you must complete the Form 479 and Letter of Agency to MOREnet. If you do apply for other E-rate services, this does not affect your ability to do so.

A new Form 479 is required every year, whereas the Letter of Agency is valid for a three-year period. The current Letter of Agency is valid through the 2011-2012 funding year.

Each E-rate eligible member must submit a Form 479 to MOREnet every year.

Letter of Agency for E-rate Year 2012 (July 1, 2012-June 30, 2013)

The current Letter of Agency is valid through June 2012. MOREnet needs a new Letter of Agency for E-rate Year 2012 (July 1, 2012 - June 30, 2013). Please send your new Letter of Agency to MOREnet by June 30, 2012. The new Letter of Agency will be valid until June 30, 2015.

Use the appropriate link below to create a letter of agency that can be printed on your organization's letterhead. Only the first page of the letter must be on organizational letterhead. Subsequent pages can be printed on plain paper. However, be sure to send all the pages.

You must have Adobe Acrobat Reader 8.0 or higher to use this application. Get the newest version of Adobe Acrobat Reader. On the following pages:

1. Select your organization.
2. Adjust the document margins to fit your organizational letterhead.
3. Click Get Letter of Agency.
4. The application will generate a PDF file. Make sure your organizational letterhead is loaded in your printer and click the printer button or click Save a Copy.
