Form 479 Instructions for Public Schools
Note: Public libraries should follow Form 479 instructions at http://www.more.net/services/e-rate/app/yr2003form479instructions-library.html.
For E-rate Funding Year 2003 (July 1, 2003 - June 30, 2004)
The Children's Internet Protection Act (CIPA) applies to all E-rate eligible organizations that receive Internet access through MOREnet. MOREnet applies for E-rate discounts on the data connections, on behalf of DESE and Internet Access available to K-12 TNP participants and other eligible affiliated organizations.
Who Must Submit the Form 479 to MOREnet
- All public and private K-12 schools and public libraries that are a part of a consoritum (e.g. MOREnet), even if not compliant.
- Schools and libraries submit the Form 479 even if their organization did not submit an E-rate application.
If Your Organization is NOT CIPA Compliant
- Public schools that are not CIPA compliant and that do not submit a Form 479 to MOREnet will be billed for a pro-rated portion of the discount lost for Internet access.
- Private schools that are not CIPA compliant and that do not submit a Form 479 to MOREnet will still receive an E-rate discount on their data connection. However, they will be billed for a pro-rated portion of the discount lost for Internet access.
- The prorated portion organizations would be billed is approximately $1,100 per year per 1.5 MB of access.
- If your organization chooses not to be CIPA compliant, please write on the top of page 1 of Form 479, "This organization is not CIPA compliant" and return the form to MOREnet. DO NOT check any boxes in Block 6 if your organization is not CIPA compliant.

Return Form 479 to MOREnet, not to the SLD.

Instructions
Block 1: Administrative Authority Information
The Administrative Authority's Form Identifier is provided for you to assign your
own unique number or letter or combination of characters to identify this specific
Form 479 with MOREnet.
- Name of Administrative Authority: The Administrative Authority is the organization responsible for administration of a
school or library, not an individual. This is determined by local policy or practice.
For a school, the Administrative Authority may be the school, school district or school board. For a library, the Administrative Authority may be the library or library
board.
- Funding Year: 2003 - 2004
- Mailing Address and Contact Information for Administrative Authority: Provide the information as requested.

At the top of page 2 is a block requesting the Name of Administrative Authority,
Name of Contact Person and telephone number. Please re-enter the information as
it appears on page 1.
Block 2: Certifications and Signatures
- Check the box
that describes the status of CIPA compliance. You can check only one box in item 6.
- Check 6a if your organization complies with CIPA.
- Do you have a filtering technology installed on ALL computers that access the Internet that block access to images that are obscenity, child pornography
and materials harmful to minors? Public libraries are not required to filter and so this requirement would not apply.
- Do you have an Internet Use policy that addresses the following?
- Access by minors to inappropriate matter on the Internet and the Web;
- The safety and security of minors when using electronic mail, chat rooms,
and other forms of direct electronic communications;
- Unauthorized access, including so-called "hacking," and other unlawful activities
by minors online;
- Unauthorized disclosure, use, and dissemination of personal identification
information regarding minors;
- and Measures designed to restrict minors' access to materials harmful to minors.
- The Internet Use Policy must be adopted after holding at least one public
meeting or hearing.
If you answered yes to all three items, then you can check 6a. Do not check any other box in item 6.
- DO NOT check 6b. This option was only available during the first funding year
of CIPA requirements.
- DO NOT check 6c. By participating
in the K-12 TNP, discounts on Internet access services are requested and therefore
this item does not apply.
- DO NOT check 6d. This item does not apply for this funding year.
If your organization chooses not to be CIPA compliant, please write on the top of page 1 of Form 479, "This organization is not CIPA compliant" and return the form to MOREnet. DO not check any boxes in Block 6 if your organization is not CIPA compliant.
- Signature of the authorized person at the school or library is required to certify this
information.
Note: Return Form 479 to MOREnet, NOT to the SLD.
